提交基本資料後，我們的註冊社工將會與您聯絡，進行一次免費的 15 分鐘電話諮詢，初步評估您的家庭照顧需求，並指引您以下哪一項專業服務最能切合您的需求。

重點諮詢服務 (單次) — HK$800 / 小時 - 適合： 處理突發疑難、查詢特定資源或尋求專業中立建議。 - 內容： 1小時專業諮詢 + 一星期內得到書面資源建議書。

三個月專屬導航計劃 — HK$3,000 / 3 個月 - 適合： 面對複雜轉變（如離院過渡期）、新確診病患或海外遠距離照顧。 - 內容： 4小時靈活諮詢時數 + 制定個人化照顧計劃 + WhatsApp 持續支援 + 第三方服務對接。

Upon submitting this basic information, our Registered Social Worker will contact you for a free 15-minute needs evaluation call. We will help untangle your current family caregiving challenges and advise you on which of the professional services best fits your needs Focused Consultation (one-off) — HK$800 / hour - Best for: Handling urgent issues, inquiring about specific resources, or seeking impartial professional advice. - What’s included: 1-hour professional consultation + a written resource recommendation report within one week.

Three-Month Navigation Plan — HK$3,000 / 3 months - Best for: Complex transitions (e.g., post-discharge), newly diagnosed conditions, or overseas long-distance caregiving. - What’s included: 4 hours of flexible consultation time + a personalized care plan + ongoing WhatsApp support + coordination with third-party services.
